Blank Application forms for use on a typewrite can be obtained from the Registrar. † Applications can be prepared directly on the computer and this is the preferred method. Go to our Application website for a copy of the Computer Word Processor Application Form. Two original typed application forms, signed, notarized, and endorsed by a proposer and a seconder are required. Direct descent must be shown from a qualifying ancestor. †Copies of documents proving proof of service and proof of the applicant's descent (birth certificates, death certificates, wills, deeds, census records, and other reliable secondary evidence) must be submitted with the application.

Two original application forms must be filled out using a Typewriter with a Black Ribbon or original computer printout on archival paper (or 100% Cotton or Rag content). †Do NOT make carbon copies or photo copies to submit as your application papers! †Handwritten or Hand Printed applications are NOT acceptable.

For those using the Word Processor Based application be sure to save the file as SR_Application_(your name).DOC. Be sure to save in .DOC format is you are using Word 2007, 2010 or 2013. (Please do not used the .docx format). †After you feel you have the application complete please forword a copy of your application as an attachment on an email to the Registrar. The Registrar will then edit out any unused generations and some other editing to remove any sections not used and check that the formatting has be preserved and then will return the file to you. If you need more area for citing proofs put in what you can and then send to the .doc file to the Registrar and he will add and edit as needed to allow for further inputting and then return the file to you for completion. †When you are complete you should send a final copy of the file to the Registrar who will keep that file for the society. †You are preparing a document that should reflect well on you and the society.
Be certain to make a photocopy of the completed application form for your personal records! This is very important!

Direct descent must be shown from a qualifying ancestor, and PROVEN by copies of birth certificates, death certificates, wills, deeds, marriage records, census, estate and Bibles records. †Generally reliable secondary source materials such as documented family histories are acceptable when copies of the original sources documents are not available. †"Hearsay" as well as copies of memberships in other lineage papers are NOT acceptable documentation!

Only one set of proofs (documentation) is needed along with the two completed original application forms. †If your brother, father, uncle or grandfather is or was a member of this society, then only a copy of your birth certificate, and other documents to show your lineal relationship to the person who was granted membership in this society using the same line of descent.

Please SIGN BOTH forms in the proper places and have them notarized. ††You will need the signatures of two members of this Society; one serves as your proposer, and the other as your seconder. †Society Officers are glad to serve as one of these for you, if you don't know of anyone to sign for you. †If the registrar is notified in Advance that a member of this society will serve as your sponsor then you may type their name in that spot along with the their SR membership number.

When your application forms are completed (proof read them carefully, the Registrar will read them very carefully), send the application forms AND one set of documentation along with a check for $60 (the $30 Application fee, and $30 for the first year dues-always check as dues may have increased since this page was written) for a regular annual membership to:

Any new or existing member in good standing can convert to a Life Membership by making an addition payment of twenty times the annual dues (currently $30 x 20 = $600). †You can become a Life Member of the SR-Wisconsin and spare yourself the trouble of having to pay dues each year and at the same time protecting yourself from future increases in membership dues. Life Members how ever could be subject to any special assessment the society may make on the entire membership. (Please check as to the current dues which may have changed since this page was posted).

Good luck in getting your family research completed, documented and getting two original copies of the application forms filled out correctly. Do not procrastinate in getting the forms filled out. Do this without delay.† If you delay and put off doing it, you will lose interest and reduce your chances of obtaining membership member in our society.
